In consideration of being allowed to participate in any way in the Alpha Training Facility /
Select Sports Club LLC fitness program, I hereby waive, release, and forever discharge Alpha
Training Facility, Select Sports Club LLC, its owners, officers, agents, and employees from any
and all claims, demands, losses, or damages on account of injury, including death, damage to
property, caused or alleged to be caused by the negligence of the releasees or otherwise while
participating in activities, using equipment, or being present at the facility located at
8278 Hwy 282, Alma, AR 72921.
I certify that I am in good physical condition and have no medical conditions that would
prevent me from safely participating in fitness activities. I understand that fitness activities
involve inherent risks of injury and I voluntarily assume all such risks.
By submitting this form, I acknowledge that I have read and fully understand this waiver and
release of liability, and I am signing it voluntarily.
